In Magic, you have to create an NPR report using computed fields to put the responses to the various queries on the report picture. I am attaching ours but you would need to modify it to use your query names from MSP CDS. Ours prints when an admission record is filed. To get this to work, you have to attach the form in admissions under the data processing, Customer Defined Parameters, Forms and Embossing settings for each admission type you want the form to print for. Our NPR report name is ADM.PAT.zcus.adm.form8 and then item #8 is set as the Medicare Form in the form menu in admissions. I am assuming that this file naming convention "ADM.PAT.zcus.adm.form?" is a Meditech requirement since I couldn't find any parameter setting this.
