That's what we do, too. In the pharmacy, we don't have access to the nuances that are involved with "holding" medications. Hold for just this dose? Hold forever? Hold until the lab results come back? Since "Hold" is such an ambiguous term, it is prudent to equate it to "D/C" until you get clarification via a "continue" etc. order.
