Hi Michele, 

We are having the same issue in regards to admissions creating
additional visits and the documentation being placed on the wrong
account.  We met last week and involved all departments and discussed
our issues.  We concluded that we needed to review and make
modifications to our current registration processes.  Also, we are going
to create a task force to review and implement these changes.  For the
interim, we are utilizing the copy routines within MEDITECH that migrate
the data from one account to the correct account.  However, it is rather
time consuming.

We discussed creating a CDS that will stay with the patient's account;
this CDS would include the affected departments and any other
information we feel applicable.  They would complete the CDS with their
information and contact the next person for them to make their changes.
Until we educate our staff, review our registration processes, these
issues are going to occur.  

Since we are in an EMR/Hybrid world, it is very hard to create and
implement a concrete solution.  If you don't mind, please post any
responses you receive from users on the MEDITECH L.  We are very
interested to see what other facilities are doing to either resolve this
issue or create a work around.
