We, we too have used Access Eforms for over two years, and I already have the UB-04 and new 1500 forms finished and are testing the form now. No ELF chips here! We also print our checks, wristbands with barcodes, all of our registration forms, etc.

I wanted to let everyone know there is an alternative to the ELF chips dilemma that we are having in regards to UB04 and finding a vendor to provide this service. Access E-forms has a solution for printing the UB04, 1500's, Payroll Checks and many other forms. If your as interested in replacing your outdated ELF chips as we were then go to their website www.accesseforms.com . We are also looking to use them to print our billing statements. Their software captures the data as a print command and uses your meditech data to print directly on forms you design and redesign as you need. Just thought I would share this product with everyone as we approach the UB04 deadlines and ELF chip vendors expensive are becoming increasingly harder to find.
