We require PRN reasons and have had it in live since before our 5.5 SR2
upgrade.  Entering PRN reasons is a 2 step process for POM users in SR2
(I think they are fixing for SR4) and our users do it but really hate it
since we upgraded.    We have since added all standard 1:1 matched PRN
reasons to the drug strings (ie Zofran prn is always nausea) so users
don't need to get stopped and enter the prn reason.  

You can really only add prn reason to the strings where it is a 1:1
match because the prn reason only shows to the user once they have
selected the string. (It shows in the green area that restates the
order).  If you have two strings with different PRN reasons they look
identical to the user.

You might want to try in test and see what it seems like to your users.
