We had the same need here to hold a room (and have it viewable on the
public tracker) for not only ambulance arrivals but also for dirty rooms
or maintenance.  So, we had a custom done for us by Meditech.  We have
not gone live with it yet.  However, it works well in TEST so far.  It
uses the existing Pre-Call Management feature to specify an unidentified
patient which you alluded to.
  
The Call Management Patient name field is used to identify the EMS
company and unit number as well as the chief complaint.  When you hit
enter it prompts to ask if this is a temporary new patient.  You click
Yes and it will then ask for the Sex and Age.  The Patient Name field
automatically gets updated with the Sex and Age.

The custom consists of the following: 
1. Allow a Room to be reserved without assigning a patient
2. Assign a Reserved or Dirty status without entering a patient
3. Ability to launch into this new Room Reserve Routine from the Call
Management Routine.  
4. Ability to view Room Availability via the Room Look-Up routine.
 
A new field is added to the Pre Call Management routine labeled "Enter
Room Hold?".  A "Y" response to this prompt launches a new screen that
includes the room number the availability type (i.e. Reserved, Dirty or
Clean) and a Comment field.

The room field has a lookup to the Room/Treatment area dictionary that
now shows the Room Availability/Condition.  The availability field is a
choice field with three choices: Clean, Dirty, Reserved. 

The comment field is a free text field that holds up to 40 characters.
After determining a room, the default comment becomes the "Caller" field
comment from the "Enter/Edit Incoming Call" routine (The call should be
documented before the room assignment).  Based upon this, if the Caller
field is filled in with the EMS company, unit number, age/sex and chief
complaint in the "Enter/Edit Incoming Call" routine, then the Room Hold
routine window and Comment field might look like this:
Room:          ER01 
Availability:  Reserved
Comment:       EMS 123 62/M Stroke 
 
We have added a second line to our public tracker so that next to the
room number we can display the Room Availability and Comment.  This will
allow the ambulance driver from EMS unit 123 to quickly see that his 62
year old male stroke patient should be taken to room ER01 without having
to find a staff person when things get hectic.  It also alerts the RN
assigned to ER01 to prepare for the stroke patient.

Maybe Meditech will make this part of their standard EDM module for a
future release.

1. A frequent flyer with a certain history visits the ER. When the
patient comes back in we want to be alerted that there is this
information on the patient- off the tracker.
 
2. We would like to reserve a room for an ambulance patient without
getting a V number or putting a name on the tracker.
