HELLO TO All on the "L" 

Currently we have a DVT screening tool on Admission, that
has most all of the risk that the literature refers to.
However, we do not have this as a weighted score.  Thus two
things are happening:  It looks like more patients are at
risk than possibly is the case, if it were weighted, thus
nursing and physicians do not take it seriously.

Therefore I'm interested in hearing from others on the
following. 

1.  On Admission is the DVT screening tool you use a
weighted tool placing the patient at low/mod/high risk? 
2.  How to you rescreen patients at some point in their
hospitalization, ie change in condition, surgery, new acute
diagnosis

3.  Is your rescreening tool different than your admission
screening tool, or does original information carry over to
continue to feed into it and just calculate a new score?

4.  If your facility has OB and Pediatrics do you have a
different screening tool for these patient populations? 
5.  If you are up on nursing documentation, did you put the
screening tool in meditech, or did you choose to place on
paper with the screening tool followed by potential
Physician orders?  

I appreciate your time in letting me know.  Any screen shots
or processes you have in place that have helped your DVT
screening be more successful are wanted.
