Arthur, there's a nice tool at www.amda.com/tools/clinical/DVTdvtform.doc that would be easily convertible to a CDS. The hospital I'm working with right now is considering implementing only the highest risk (5 point) questions as their screening process for now, and putting it only on the admission assessment. So of course, I'm also interested in other people's answers to the question about reassessment. On a related note, the only DVT/VTE related protocols I've seen were based entirely on the results of the INR/PT. I'm curious to know if anyone has a protocol that is driven by something else. Sharon
