We had the same problem. We finally got a custom that if the nurse enters an amount in the dose field on 0 dose orders that is greater than what we have set in the Pharmacy Customer Defined Parameters In the Maximum Dispense Quantity field, they get a warning as to whether they want to continue (no Default). If they say Y, they then get another warning telling them they are documenting nnn units, do they want to continue (with no default). If they say Y, then they have to enter a reason. (This may not be it exactly, but pretty close.) We had to do it because when they just got a warning, they went right by it. Since we have had this, we have not had the issue with 0 dose orders.

I have a report that I got from Bill Mullins that lists charges over a certain dollar amount. I have it scheduled to print every morning and list all charges over 12,000. This is not for every med. It happens with our 0 dose and with Non-formulary meds( we have a med named non-form and we put the med into the dose instructions.) I run a report everyday to check for errors. I also taught the nurses how to run an admins by user report so they can simply check their meds for any big numbers. They do not all do it and I am the one worried about what the chart will look like. What are others doing about this? I don't want to send out a mean message telling them this could affect their license and it is also not fair to them because I am not able to program something into meditech to prevent these kinds of errors.
