I have been asked the following questions.....anyone who can help , it
would be greatly appreciated.....

Could you do an inquiring on the Meditech L to see what other ICUs are
doing for documentation of I/Os and the times that they calculate.  
Questions are how are you handling the following .. are you documenting
in MEDITECH...or manually on paper , if you are documenting on line how
are 
you doing it ?
 
1. Titrations , drip titrations ?
2. Fluid changes, etc. We keep track hourly on the flow sheet each drip
and the actual volumes, 
3. NG, PO, Blood products, IV medications, etc. 
4. We record vitals and the above items  trackable by hour, individual
category, and 24 hour total based of a 6am start time.  According to
Meditech, the entire fluid plan of care will need to be looked at since
Meditech bases everything off 12 midnight as a start time. How are other
institutions doing this , are they using 12 midnight and all physicians
agree with that ?
 
I would like to see if any other ICU's have been able to utilize the on
line I&O and the simplicity of it.  Last I knew there were very few
engaged.  I do not agree with doing it on flow sheets and making the
nursing staff double document every 4 hours either.

I had a conversation with a provider yesterday regarding the inablity to
track intake and output easily due to the confusion of two different
systems between ICU/PCU and the med surg units. I told him I would
investigate moving the ICU/PCU toward online documentations of intake
and output. Can this happen rather quickly. Frankly I have to agree that
this is less than optimal and jus another barrier to assuring the I&O is
completed and timely by all staff.
