We are trying to find a more user-friendly way to manage care plans than process plans which seem very time consuming and complicated for the nurses to actually use as they should. We have thought of building interventions instead and actually have one built as a plan of care ( I inherited it when I came to the job) but I am unsure how to prioritize as JCAHO wants and also how to make make them flexible enough to individualize for each patient. I am looking for a way to make this easy for the nurses so that they actually document as they should all of the time. Anyone doing anything like this? Any input would be appreciated very much.
