We just had a custom delivered that warns the user if there is a doc with a similar mnemonic. It will not be fool proof by any means but it will help. We had to pay for it.
We are trying to get our folks to use the convention of N \ xxx (first 3 characters of the docs mnemonic) as the method of selecting the doc in any doctor field (Lab and OE anyway). This way, all the docs with the same last names will be grouped together instead of being spread out between pages when doing a normal doc lookup. It is really slick if folks will use it. This combined with the custom should minimize our risk occurrence (incident) reports. We decided there is no "system" fix for such a problem. The users are just being careless. We used incident reports and relied on our compliance department to correct the issue.
