Hi there,
At this facility we have experienced this issue. We have tried two
different ways to handle it.
1. The Pharmacist puts in the higher dose and the nurse documents
why she is giving a lower dose to be nursing judgement. And that the
range is 5-10 mg and that she is only giving 5.
2. The Pharmacist puts in both a 5mg dose and a 10 mg dose and the
nurse picks which one to use based on the dose she is giving.
