Here's what our nurses gather on the initial assessment.  It's set up to
calculate a value for each answer, default the total for the assessment
score, and then default the risk factor in relation to that score:

2. Secondary diagnosis or more than 1 medical diagnosis:
3. Ambulatory Aid:
4. IV Therapy / Saline Lock:
5. Gait:
6. Mental Status:
  * Assessment Score:
  * Fall Risk Factor:                            Initiate Safety Risk
