Theresa, The only place that you will find the old information referenced for documentation in NUR is where it is saved in ADM.PAT. If you actually look in NUR you can reference each individual instance of documentation and see which queries were filed blank on each date/time. Because it is so "easy" PCM's have resorted to referencing the information stored in ADM.PAT rather than that stored in NUR.PC.WORK. There are a couple of ways around this issue. One is to make the query demo recall "Y". In addition to saving the information in MRI so it is available from visit to visit, this also makes it save the blank or nil information. You wouldn't want to do every query this way, but this might work for specific queries.
Another thing is to look at how you have written your profile. You can set up a profile that pulls erroneous information but labels it in a way that staff, HIM, Quality/Risk and Lawyers would think was current and accurate. So, you need to understand the implications of each of the choices within the profile dictionary. I reported this to Meditech and was told "Filing the screen blank without default queries (don't go there) would not overwrite the previous response as the system cannot tell if you are intentionally filing this blank or just skipping the query. As such it would not overwrite the previous response."... needless to say I don't like this answer so I am asking the experts. As a work around I have created an NA response but now have to educate the users to place "something" in the response so the previous response is overwritten. This is a meaningless step for the nurse in my opinion. I am sure we are not the only place this is happening to... any and all suggestions are welcome.
