If you go to ADM.PAT.ccdqr data for NUR queries, here is how it works:

1) If the Intervention is set to "Default Queries" = N
Only non-nil responses "file back" to ADM.  This means if you have an
old answer, and then later the user deletes the answer, the old answer
persists in ADM ccdqr.data

2) If the Intervention is set to "Default Queries" = Y, a nil response
in a subsequent documention will delete the prior answer in NUR.
Additionally, if you undo the most recent documentation, all the
corresponding queries in ADM ccdqr are deleted.

For this reason, I think it is better to get your NUR query responses
from NUR, from the most recent time documented, post edit, skip undo.

When a site has multiple interventions where they might have documented
a query, we pick the most recent intervention for the patient and use
the answer on that intervention/date/time/ctr (always post-edit, skip
undo)

I don't know what they were thinking with this design, but I think they
stick things in ADM.PAT.ccdqr just to make the recall feature
convenient, not as any kind of "here is the most recent answer" feature.

Unfortunately, MT R/W group and others have been treating the ADM data
as "most recent answer", indeed I wrote a few reports that way until I
noticed the odd behavior of queries that would be in ADM but not in NUR.

Good Morning,
We are a Magic shop currently 5.4 SR2 going to 5.61 SP6 next week.
We have been up with NUR documentation for 5 years now. 
 
Issue/Question:
In the NUR module:
 When a field in an intervention has a response....  (query)Current
Legal Status:
(response)Form 52
In 3 days the Form 52 is lifted and nurse files the CDS with the
response blank, the Form 52 response still shows on the Patient Profile
and in a header report I have for VFS. (I don't see the old response on
the VFS). 

I reported this to Meditech and was told 
"Filing the screen blank without default queries (don't go there) would
not overwrite the previous response as the system cannot tell if you are
intentionally filing this blank or just skipping the query. As such it
would not overwrite the previous response."... needless to say I don't
like this answer so I am asking the experts.

As a work around I have created an NA response but now have to educate
the users to place "something" in the response so the previous response
is overwritten. This is a meaningless step for the nurse in my opinion.

I am sure we are not the only place this is happening to... any and all
suggestions are welcome.
Does it work this same way in other applications?
