We started out with 2 forms - one history and one admission assessment.  It got 
to be confusing and redundant at times on the amount of questions.  So we 
combined our forms into one assessment - Pt Admission and History.  Now our 
policy says that LPN's can do admission assessments - due to the length of the 
form and time involved, but the RN is responsible for cosigning those 
assessment forms and verifying that information.  Actually, most of our 
admission assessments are still done by the RN's.

Our LPN's document general information.  They are not allowed to do the
physical assessment.  We have broken our assessment into two parts.
Part one is done by the LPN this does not generate the careplan.  Part
two that is done by the RN is the one that generates the care plan.  All
of the answers that are on part one default into part two for the RN to
review.  If you want let me know and I will send you screen snap shots
of our assessments.

For those of you out there that use query links to add diagnosis to the
care plan, how do you accomplish adding a care plan on the admission
assessment by triggering diagnosis if the nurse is an LPN?  Do you have
an RN review the care plan chosen by the LPN and document a note, etc..?
