I am interested to hear if anyone else has tried a CD image (not a real CD, 
just pointing QEMU at the .iso).  It worked for me although neither USB nor 
RAW worked.  

But if CD doesn't work for others then there must be some other difference in 
the way I built it and I will go back and try to work out what is going on.

As I mentioned in my earlier email in this thread, I found it necessary to 
specify -no-acpi on the QEMU command line.
