You might find the Maemo packaging tutorial useful: http://wiki.maemo.org/Packaging - you can ignore all the dpkg stuff.
The important bit is the .desktop file install. I don't know how many Maemo extended fields are also in action for the handset UX, I imagine most of them. The important bits are the Icon field, ensuring that the icon is installed in the right place, and the Exec field, identifying the executable to be run.
You might also try compiling & running your application under Xwindows first - perhaps you're missing some vital step in the initialisation of your application (the Xlib equivalent of QWidget::show() perhaps?)
