Applications must be received by November 3rd (postmark doesnt count), accompanied by a $50 check made payable to CSMFO. The application forms can be downloaded from the CSMFO website or transmitted via email attachment by sending me a request. Submit one $50 fee for each category you apply to. No submittals will be accepted after the November 3, 2000 deadline. There will be no exceptions. Again, a postmark date does not qualify as received date. However you plan to ship your materials, make sure you provide adequate time for the shipping option you select.
