The C changes are generally pretty trivial to accomodate in the
extensions I have looked at.

Mongrel _almost_ works out of the box on 1.9.1.  It just needs a
couple very simple Ruby changes, and it will work.  It's been a couple
months since I did it last, but IIRC, it's just removing the fastcgi
requirement, and changing a case statement's syntax to conform with
the 1.9 syntax changes.
