I'd clear away all the loose stuff, as I find a larger clean hole, with a clean edge is easier to patch. You can get a bag of ready mix driveway patch repair. You have to over fill a bit, and then tamp it down well! If you do it right, and it's not hard, you get a pretty good, durable repair. As I said, don't be frightened to do as the directions say, and tamp it well. You can rent a tamper, or if the patch is not too big, a piece of wood and a 5lb sledge work ok in a pinch. Once done, you can leave it as is, or seal over it. You can find it at places like HomeDept, for about $20 or so a bag.
