Ok, so no fan clutch:) Now I need to understand why a clogged radiator or
ailing water pump wouldn't cause my car to run hot at lower speeds or while
idling?

Bingo! Either the radiator is clogged, or the coolant is not circulating.
Cheap aftermarket water pumps have been rumored to have the impeller blades
rot off.

Dimitri has a 240D.
240Ds don't have fan clutches.

Some of the fan clutches DISENGAGE the fan at highway speed. Even if they
don't the fan is effectively doing nothing at highway speed. There is way
more air being slammed over the radiator by the car moving forward than the
fan could ever move.

At highway speed you don't need a fan at all.

Thus you're wrong, a bad fan clutch will not make the car overheat at
highway speed. Or rather it won't be because the fan isn't spinning, there
might be another factor at work.

As an aside 190Ds have electric fan clutches, they will always be
disengaged when the engine is off...

The obvious question is "have you checked the coolant level"? I
presume the answer is YES and the level is NORMAL.

If so, with the engine off spin the radiator fan. if it spins freely
without reistance the fan clutch is worn and needs to be replaced. A
worn fan clutch will cause the engine to run hotter at highway speed
than at slower speeds. At least this was the case in my W108 sedan
and W111 3.5 coupe.

How often was the coolant changed and was the engine ever overheated?

Even with genuine MB coolant if you never changed it and then overheated
the
engine a couple times you'd probably blow a headgasket...
