Head gasket was replaced last month.

I'm betting on the block but will probably never know.

Why would you run a car without the cap if nothing else is wrong?  If there
is nothing else wrong the temp should never reach the boiling point of
water let alone an antifreeze mix.  Basically if your Diesel is idling hot
enough to boil over I am betting the cap is not the only problem.

  The problem with the car in question is obviously not a temp issue but a
pressure issue.  Bad head or block, maybe blown gasket leaking cylinder
pressure into the cooling system is more likely.
