I have a way to duplicate this: Download this area: http://www.openstreetmap.org/?lat=53.3794&lon=-8.9711&zoom=14&layers=B000FTF Join way_28530421 (waterway=river) to way_28530422 (waterway=stream) The end of the stream (at Augcloggen) will be joined to to river, rather than the closest node. Note that if you reverse the direction of the stream, then the correct node will attach to the river.
