I've only been able to make it happen when the server is local to my
machine. I'm playing on a PII-400 with a V3 board. The config is
attached to this message. I have the quality settings up as high as
possible.

v1.03 or v1.05 (the latest) I didn't see it with IHVTest, but I didn't
work that one as hard either.
