in my previous lifef i programmed on 4th axis all the time. I set a user command at a safe z return level as the first tool element in my model. I set my machine define and post to use z home and this worked fine. the tools will retract to this z level before each rotation. using this allows you to use local planes for programming; z zero at 0.0, clear at .1 , etc. the code output from word rather than local planes also.

I think you need to have a clearance point high enough to clear all rotating
parts as the very first element in your model
