is missing it is assumed to be the page, so maybe there needs to be away
to represent that?

I agree that it's optional, but I don't think that means it ceases to be root. Presumably the presence of *a* root is required, but that's covered by the page assumption.

So I think it should be bold, but not underlined. But I'll not be losing sleep over it.
