Possible reasons include:

1) mod_midgard is not loaded
2) MidgardEngine is off
3) ServerName doesn't match the host record's name

You can check these by setting the Apache LogLevel to debug and trying
again.

1) If you don't get lines like "Midgard: database=..." and the server
version string doesn't contain "Midgard/1.2.4" then you should make sure
that you have the LoadModule mod_midgard.so directive in your
httpd.conf.

2) If, when requesting a page, you don't get a line like
"midgard_translate_handler(...)" then the MidgardEngine directive is off
and you need to set it on.

3) If you get lines like "host record for ... not found" then there no
host record contains the name and port you have defined in httpd.conf.
This can be fixed by modifying the host record in the database.

If none of these hints help, then you might want to send a piece of your
error_log for closer inspection.
