Make Define New Fee "fee calculation" options toggle

The define new fee page has two distinct options for loan fee calculations:
1 - amount
2 - calculate fee as %
Between these two options is an "OR", but the general usablity of this page
should be improved to disable the fields that are not applicable with each
case. For instance, the "Currency" field appears next to amount when
additional currency codes are added. However, currency does not apply to the
2nd type of calculation (% fee).
This is not clear to the user as several fields that are marked as mandatory
are no longer required depending on the calculation option. Requesting the
fields that are not applicable be controlled via radio button or some other
means to disable fields that do not apply.
