Summary: Create Fee should only show Calculate Fee as if Fee applies
to Loan (was: creating fee with rate calculation bug)
This is a regression. In previous versions of Mifos, "Calculate Fee As" fields
only display if the Fee type is "Loan". If the user selects other fee types,
those fields should not show up and the user should only be able to select an
Amount.
