* The .patch file looks fine.
* The database upgrade in the .sql file must be part of the patch (you
mentioned John W. said it would be fine to submit separate for now: I agree)
* A test sounds like a good idea, but there may already be sufficient coverage
of the mechanism for handling hidden/mandatory fields. Your call.
Please attach the complete patch when it's done.
Take a look at trunk r16583 for an example of adding hidden/mandatory fields.
