Kalyan Akella resolved MIFOS-3570.

Resolution: Fixed
The issue happened because the Questionnaire stories introduced the 'Smart
Select' answer type which was unsupported by the existing Surveys code base. I
modified the Surveys code base to ignore any unknown answer types while loading
all the active questions in the system.
Please verify the fix on head-master build #1877 or later.
