Scenario:
* client A was created when survey #1 was active on client creation
* survey #1 is marked as inactive
* survey #2 is loaded and is active for client creation
* go to client A page and click "View Additional Information"
* survey #1 shows up and survey #2 (with blank answers) shows up below it
Question: should survey #2 show up with blank answers for clients created
before it was activated?
