There is an issue with 'survey_questions' table because it contains too many
entries for the survey which applies to 'All'. For example if survey contains 5
questions then in the database there are 30 entries for that survey instead of
25 entries. It looks like when it applies to 'All' then it applies to 6 flows
instead of 5.
