My suspicion is that somewhere in the code, Lookup_ID and Lookup_Value_ID are 
getting wrongly used.
Please see below. 1084 is the Lookup_ID for "Project Officer" and is also the 
Lookup_Value_ID for "Anoring"

select * from lookup_value_locale WHERE LOOKUP_VALUE like '%noring%' or 
lookup_value like '%project officer%';
returns:
LOOKUP_VALUE_ID, LOCALE_ID, LOOKUP_ID, LOOKUP_VALUE:
1084    1       743     Anoring
1423    1       1084    Project Officer
