I'm pretty sure that senario was caused by a firmware bug sometime
during the 4.x series.  I did daytime upgrades while I had personnel
on site until I got them upgraded beyond that firmware rev.  I don't
remember what the revs were anymore.  I had the 493Gs and 450Gs do it
to me.  But it wasn't every unit in my case.  It was more like any unit
which didn't have a person standing by to power cycle it post-upgrade.
I think they have proximity sensors.
