This is because File::Scan is a perl module that is loaded when MIMEDefang is loaded, it's not an external program or daemon that is called per-message. Think of File::Scan as being part of your mimedefang-filter, change the filter means restart (reload) MIMEDefang. McAfee, Sophos, Trend, Clam, ... are all external and wouldn't require a MIMEDefang restart.
