Mandoc uses the exact paper size as specified by ISO 216 in millimeters:

  A3  297 x 420
  A4  210 x 297
  A5  148 x 210

For the North American sizes, it uses these approximate values:

  letter  216 x 279
  legal   216 x 356

These values in millimeters are converted to AFM units (assuming a
Times-Roman font of size 11) according to the same calculation and
rounding rules as any other lengths, using double precision
floating point, then truncating to unsigned integer AFM units.

There is a rounding difference in the A4 height given in the
gv(1) manual of one basic unit, but i don't know which rounding
rule gv uses.
