Qmail is best-known among everyone equipped with sufficient experience as the cause of numerous operational issues and a fair amount of abuse thanks to a number of very poor design and implementation decisions. Many of these have been discussed over the year in exhaustive detail on the appropriate mailing lists and newsgroups. Anyone who isn't fully aware of this simply hasn't been paying attention.
