thanks for that, that was all I needed. For the record,
1. Installed latest doxygen on Mac.
2. Made sure MITK_REVISION_ID was set, using:
mitkFunctionGetVersion(${MITK_SOURCE_DIR} MITK)
3. Made sure BLUEBERRY_USE_QT_HELP was ON
4. Made sure my external app, basically had an application plugin that was set
up like MITK/Modules/Bundles/org.mitk.gui.qt.extapplication
5. Made sure I had a <plugin>/documentation/UserManual folder with the
documentation in.
and it worked.

We do not have an over-view document for the help system, but the plugin
style guide explains shortly what goes where:

Everything is actually pretty much automated inside the
MITK_CREATE_*_PLUGIN macros. So if CMake finds a version of doxygen
installed on your system, the macros create Qt Compressed Help files
(.qch files) from the content in documentation/UserManual and compile it
into the plugin's shared library. When starting QtAssistant from the
ExtApp, you documentation should show up.

application it doesn't appear to do anything, so I'm not sure what to check.
