Just an addition. These are known bugs in ITK and VTK that have already been fixed in the mainstream:

One difference is that I had to add the include for stddef.h to Code/IO/itkImageIORegion.h as well, but I did not need to add it to Code/IO/itkConvertPixelBuffer.txx like in the linked commitdiff above.
