Last time I checked there was some development that needing doing in the kernel to fix some problems with the real time system. Not sure if this has been resolved yet.
It's worth remembering that the rt kernel has some problems with proprietary drivers such as the nvidia graphics driver as they taint the kernel and real time performance cannot be guaranteed. Nouveau (or open source ATI drivers, etc) is a required option for rt.
