Hmmm...  which version are you using? There was a very subtle multiplexing 
issue which (annoyingly) which could have the effect you describe which only 
showed up on certain players (hardware and software) and so took *ages* to 
detect.

The current stable branch (which should shortly be released as 1.6.2) has the 
bug fixed.
