Improved version.
Find attached a diff from r18564 which will patch ProxyImpl and ProxyExt,
and a new class to hold the termination info.
This now does not depend on originalRequest being stored, but creates new
clean BYEs from scratch.
We have tested this on both a single server and a dual-redundant pair, and
the fail-over is working.
Not had time to look at writing a regression test for it yet.
