I'll get back to you on the documentation.... :)

To clarify #5, the app template assumes I'll be running my app from
http://foo.com/bar, where bar is the name of the application.  However, if I
want to run my new app from http://foo.com/ there's no way to have an
"unnamed" app... or to be more clear, an application which runs from the
root.
