You have encountered the same bug I reported last month.  What happens is
this:

a) Apache starts up and prompts for the passphrase
b) Apache spawns a child process, which also asks for the passphrase
c) Because of a bug in Apache, the prompt string is not displayed on the
console, and the child is hung waiting for the passphrase

Solutions:

1) simplest for testing - just type the passphrase a second time
2) also for testing - start Apache with the -X parameter; this means Apache
does not spawn a child and hence prompts only once
3) write a program to emit the passphrase and use the SSLPassPhraseDialog
directive
