We abandoned using extended database because many of the mailbox settings wouldn't get populated automatically, or from reading the list others had problems with the upgrade with their existing settings not populating correctly or something to that extent. I would almost bet that if you go set the spam settings from the console to a domain/mailbox, and 'apply' that, it will start working. Just a guess...
