FreeBSD and OpenBSD used to be a PITA. The Apache never had threading
compiled into APR and so daemon mode not possible and only preform
embedded.

Even then, to get threaded Python working in prefork required having
force Apache to preload appropriate threading library. I believe
things are better these days, but don't know if they have solved all
their threading stability issues or not.
