You seem to have covered most of the points.  The only other possibility is 
email address spoofing but I doubt that is the case here as if you got a mail 
from him it is no doubt using his address book to mail all his contacts.

The email address I have for him is a btinternet.com one and although it's 
possible this has been compromised it's also possible he has picked up a trojan 
of some description.  If he changes his password on the account he should also 
check that any other security questions, if there are any, and alternative 
email address for confirming password change are also checked and updated.  A 
full system scan with updated virus definitions is essential.
